The normal scale for hemorrhoid classification is the Banov system. This hemorrhoid classification system ranks the symptomatic states on a 1 to 4 with grade 1 being the least extreme. A grade one hemorrhoid is a real internal hemorrhoid; it does not protrude, even during defecation. A second grade hemorrhoid would usually protrude beyond the anus during defecation. However the grade 2 hemorrhoid will reduce spontaneously and withdraw to the interior of the sphincter if not irritated. A 3rd grade hemorrhoid needs manual reduction, often accomplished by squeezing. 4th grade hemorrhoids have strangulated. The tissue is dying because of lack of oxygen and must be surgically excised.
The cause of the original internal hemorrhoid may be portal hypertension, chronic bowel problems or diarrhea, pregnancy, obesity and poor muscle tone.
